Kidney Stone and a High Sugar Diet

Our kidneys are bean-shaped organs, each about the size of our fists. They are located near the middle of our back, just below the rib cage. The kidneys are sophisticated trash collectors. Every day, our kidneys process about 200 quarts of blood to flush out about 2 quarts (fourth part of gallon, two pints) of waste products and extra water which together become urine that flows to our bladder through tubes called ureters. Our bladder stores urine until we feel urge to go to the bathroom.

The wastes in our blood come from the normal breakdown of active muscle and from the food we eat. Our bodies use the food for energy and self-repair. After our bodies have taken what they need from the food, waste is sent to the blood. If our kidneys do not remove these wastes, the wastes would build up in the blood and damage our bodies. In addition to removing wastes, our kidneys help control blood pressure. They also help to make red blood cells and keep our bones strong.
 

Many ions circulate in our bloodstream, some combinations of which can precipitate to form kidney stones. Kidney stones, or renal calculi, are solid concretions (crystal aggregations) of dissolved minerals in urine; calculi typically form inside the kidneys or ureters. The terms nephrolithiasis and urolithiasis refer to the presence of calculi in the kidneys and urinary tract, respectively. There are four major types of kidney stones:
1. The most common type of stone contains calcium that is a normal part of a rich diet. Calcium that is not used by the bones and muscles goes to the kidneys. In most people, the kidneys flush out the extra calcium with the rest of the urine but in some people it is accumulated in their kidneys. The calcium ions that stay behind in kidneys join anionic waste products such as oxalate and phosphate ions to form a stone.
2.  A struvite stone may form after an infection in the urinary system. These stones contain the mineral magnesium and the waste product ammonia.
3.  A uric acid stone may form when there is too much acid in the urine. Those of us, who have acquired tendency to form uric acid stones, should vow to cut back on the amount of meat they eat.
4. Cystine that is a sulfur-containing amino acid produced by digestion or acid hydrolysis of proteins, and sometimes found in the urine and kidneys, and readily reduced to two molecules of cysteine, can build up in the urine to form a stone. Though cystine stones are rare, the disease that causes cystine stones runs in families.

Kidney stones may be as small as a grain of sand or as large as a pearl. Some stones are even as big as golf balls. Stones may be smooth or jagged. They are usually yellow or brown. A stone may stay in the kidney or break loose and travel down the urinary tract. A small stone may pass out of the body without causing much discomfort. A larger stone may get stuck in a ureter, the bladder, or the urethra. Problems arise when stones block the flow of urine and cause great pain. Some stones can become large enough to be extremely painful and even life-threatening, requiring treatment by drugs, lasers, or surgical removal.

It has been discovered that kidney stones generally consist of insoluble calcium and magnesium compounds such as calcium oxalate, calcium phosphate, magnesium ammonium phosphate, or a mixture of these. For calcium oxalate kidney stones, the following equilibrium applies:

CaC2O4(s)           Ca2+ (aq) +C2O42-(aq)

High intake of foods rich in calcium or oxalate can cause a rise in the urinary concentration of either ion or both, sufficient to shift the equilibrium to the left and cause the ionic product to become greater than the solubility product that is the product of concentrations of ions in a saturated solution of a highly soluble salt, each raised to the power of the coefficient in a balanced equation. The result is precipitation of calcium oxalate as a kidney stone. Thus, foods rich in calcium, such as milk, ice cream, and cheese etc., or high in oxalate ions, such as chocolate, spinach, and black tea, and likes, can trigger the onset of a kidney stone through the common ion effect (A common ion is an ion that is produced by more than one solute and the shift in an existing ionic equilibrium caused by the addition of common ions is called the common ion effect). Such foods are restricted in the diets of individuals prone to developing kidney stones. A high sugar diet may also create kidney stones because excessive sugar promotes excretion of calcium and magnesium ions, which increase the concentration of these ions while passing through the kidneys.

In order to avoid both the onset of kidney stone and formation of more stones one should drink more water. In fact we should try to drink 12 full glasses of water a day as drinking lots of water helps us flush out the substances that form stones in the kidneys. We can also drink ginger ale, lemon-lime sodas, and fruit juices. But water is the best. We should limit our coffee, tea, and cola to one or two cups a day because the caffeine may cause us lose fluid too quickly. Generally doctors ask patients to eat more of some foods and to cut back on other foods. For example, if one has a uric acid stone, the doctor may ask him/her to eat less meat, because meat breaks down to make uric acid. Besides, a doctor may give the patient some medicines to prevent formation/aggravation of calcium and uric acid stones.

Thanks to a recent Instagram story posted by the America’s Next Top Model judge, we happened to get a glimpse into one of Graham's evening exercise routines. Let me tell you, it is a killer upper-body circuit that hit the triceps, biceps, chest, and back. And she finished off with some core work.



The best part: it only took Graham 2 minutes and 48 seconds to get through these moves. (Yep, I timed her). Granted, you have to take into account that she probably rested here and there, and did a few more sets and reps than she let us in on, but even with those considerations, this is still a great workout option for when you are short on time



Our suggestion: Cycle through this 11-move circuit 3 times. If you do that, you are looking at about a 10- to 12-minute upper body blaster that you can knock out the next time you are in the gym. Oh, and you also might want to download the playlist Graham was rocking too—Sean Paul’s “Gimme The Light,” Mr. Probz’s “Waves,” Kid Cudi’s “The Pursuit of Happiness,” Eddie Money’s “Take Me Home Tonight” and Future’s “Real Sisters”—because it was kind of fire!

Seated One Arm Row



Start seated on a bench with legs wider than hip-width apart and feet planted. Place left hand on hip while right arm is extended up on a diagonal with cable handle grasped in hand; palm face down. Squeezing back, pull rope down towards torso, twisting hand out so that palm faces in; keep arm close to body. Hold, and then slowly return back to start. Repeat on opposite side. Seated Lat Pulldown



Start seated with back straight, knees under knee pad and bar in hands (overhand grip) slightly wider than shoulder-width apart. Without moving torso, pull bar down to chest while squeezing shoulder blades together. Pause, and then slowly return to start. (Graham did 7 reps.)

Standing Biceps Curls



Stand with feet wider than hip-width apart and knees slightly bent. Hold a dumbbell in each hand in front of body; palms face up. Bend elbows and curl the right hand up toward the right shoulder. As you slowly lower the right hand back down, begin repeating the motion with the left hand. Continue alternating. (Graham did 4 reps per arm.)



In & Out



Start seated with arms behind you and hands on floor; fingers facing feet. Lean back, raise legs and bend knees. With abs tight, extend legs straight out as you lower back down slightly. Pull legs back in and lift torso back up. Continue repeating. (Graham did 17 reps.)

Jacknife

Lie faceup with legs extended straight up, a light bend in knees, feet crossed at the ankles (left over right) and hands lightly on back of head. Crunch up and then lower back to start. Repeat. (Graham did 15 reps.)

This interview is an excerpt from Kevin Gianni's Renegade Roundtable. In this excerpt, Dr. Doug Graham shares some surprising information on diet, dental health and disease.

Dr. Doug Graham is a lifetime athlete and 27-year raw fooder. He's been an advisor to top performers including tennis legend Martina Navratilova. He's also the author of the lifestyle book 80/10/10.

Kevin Great. I wanted to talk to you about tooth care because a lot of people who get into the raw food diet have specific teeth issues that may arise. What are some of the challenges that people face? What are some of the things that you've done to kind of help reverse some of that?

Dr. Graham: Tooth care, it's an interesting thing. Long ago people asked me to write about diseases. I said, "You know, I don't really write about diseases because diseases are all pretty much the same. If you stop participating in the cause, the body won't generate the symptoms." Every individual disease, there's thousands and thousands of human health conditions. I think it comes up the same with teeth. Oral hygiene and oral health is once again just one tiny aspect. It would be a shame if we were out there in the world telling people, "Look, we've got this diet. It's the best diet in the world for everybody but it's really terrible for your teeth." Well that wouldn't work. It turns out that the diet that we espouse is really wonderful for our teeth.

And obviously, but it still has to be said, because anybody that's ever been to a dentist's office has noticed that the dentist offices around the world are filled with people eating cooked food. So we can't just look at raw food and say, "Raw food is really bad for your teeth because look what happens," because we're seeing the same thing happen on cooked food. At the same time we're also seeing two other issues going on with our teeth that have to be at least allowed for. One is the fact, as disclosed in Pottinger's Cat's, that with every generation of cats eating a diet that was not species specific there was degeneration, both physical and physiological. In other words in the form and function and the design of the cat we saw decay.

We notice in human anatomy that changes have happened. One of the big ones that happens with malnutrition over generations is a loss of what is called the broadness, a narrowing of the dental arch. So for instance, if I smile at you, you might see 8 or 10 teeth whereas someone with a broader dental arch would smile at you and you would see 12 teeth or 14 teeth. The broader the front of that arch is, the wider the horseshoe, effectively, that the teeth make, the more room there is for teeth. So we're seeing an increase with each generation. We're seeing an increase in the number of children who need orthodontic work because there simply isn't enough room for the number of teeth that are coming in, so they start crossing over each other instead of sitting alongside of each other.

We have inherited not only this problem but several others, with the health of our teeth, before we ever went raw. Then we have these issues with going raw. One of them is that a lot of people go raw and have a really hard time staying raw, so they're switching back and forth between cooked and raw, which doesn't do our teeth a lot of good, especially the kinds of foods that tend to get cooked, because as you become aware on some level, cooked food is really sticky. It sticks to our teeth like crazy. If we're going to eat food that sticks to our teeth and a bunch of sweet foods as well, we're going to have problems. In fact, anything that sticks to your teeth is a problem. All cooked food is sticky, all dehydrated food is sticky to some degree, stickier than its fresh counterpart. Nuts and seeds are sticky, they've been dehydrated.

The other problem that comes in is almost invariably some aspect of nature attack us when we go raw. We become 'nature boys' and 'nature girls' to a greater degree than we ever were before. A lot of people have come to me and said, "I went raw four years ago. I stopped using shampoo. I stopped using deodorant. I stopped using a toothpaste. No animal brushes their teeth. I stopped brushing my teeth." The number of people who've told me that is just astonishing. And I'm going, "Yeah, you know, you didn't really earn that right the first day you went raw to expect that your body could function as if you had been raw your entire life and your parents had been raw their entire life and it had gone back for generations." I'm not really sure that no teeth care is the plan.

In fact, it's not the plan that I recommend. I recommend a tooth care plan. I recommend that you do clean your teeth after you eat and that you do not only a careful brushing under the gum line, or right at the gum line - this is where our teeth are most vulnerable - but that you also get out the particulate matter that's in between your teeth. Whether you're doing that with a bristly kind of round brush or whether you're doing that with floss or whether you're doing that with pick or a brush pick, there's a wide variety of ways to do it.

We eat raw food and our teeth feel so clean but in fact there's still stuff, there's still material. And organic material stuck between your teeth is going to be broken down by bacteria. The bacteria will produce acid that will harm your teeth. In almost every case, I would say in every case of dental issues that I've seen with raw fooders, it's a matter of them having decay at the gum line. It's always at the gum line. It's not the enamel portion that they're having such problems with, it's at the gum line where there isn't enamel. And this is because on a standard cooked food diet we actually cause a loss of gums but also an inflammation of the gums. When the gums are inflamed, even though we've lost some gum matter, the inflammation makes them bigger and it covers up that area of tooth that has no enamel. When the source of the irritation goes away, we start eating raw foods that are not irritating to the gums instead of the cooked foods that have irritants built right into them, the food additives all function as irritants to the gums and so they become inflamed.

When we go to raw foods and we get rid of that irritation, the inflammation also goes down at which point we start to expose the teeth at the gum line, areas of teeth that don't have enamel, and without cleaning them on a regular basis you're definitely going to have tartar build up. What happens is kind of like a pearl in an oyster; the pearl functions as an irritant. If you get a little tiny bit of tartar under the gum line. If you get a little piece of food under the gum line it's like picking up the blankets on the bed, it pulls the gum off of the teeth and then anything else can get in under there. At every meal stuff starts getting under there and the gums just get worse and worse, they recede quickly.

The gums are supposed to be dull in color, they should not be shiny. They're supposed to be sort of stippled looking. They should not be puffy looking in any way, shape or form. If your gums are puffy or shiny, if the color isn't that of healthy gums, you need to take care of them. This idea that we can live without washing is just not founded. Cleanliness is next to Godliness, as the old saying goes. Certainly cleanliness is a huge part of health and hygiene for human beings. We have to take care of ourselves.

Not an April Fool's joke: Petrol price slashed by Rs 3.77, diesel by Rs 2.91

Oil marketing companies today slashed petrol price by Rs 3.77 per litre and diesel by Rs 2.91 per litre. The decision comes following fluctuations in crude prices.

"The current level of international product prices of Petrol & Diesel and INR-USD exchange rate warrant decrease in selling price of Petrol and Diesel, the impact of which is being passed on to the consumers with this price revision," an official statement said.

The revised prices will be effective from this midnight.

Petrol currently costs Rs 71.14 a litre in Delhi and diesel is priced at Rs 59.02 per litre.

This is the first change in rates in two-and-a-half months.

Petrol price will be cut by Rs 3.77 per litre, excluding state levies, said Indian Oil Corp (IOC). Actual decrease will be larger after taking into account the local levies. Similarly, diesel rate was cut by Rs 2.91, excluding state levies.

Rates were last revised on January 16 when price of petrol went up by 54 paise. On that date, diesel rates were hiked by Rs 1.20 a litre.

IOC said the movement of prices in the international oil market and INR-USD exchange rate shall continue to be monitored closely and developing trends of the market will be reflected in future price changes.

The gallbladder is an organ that you do not need to survive, but nonetheless it can cause quit a few problems and sometimes even pain so severe that the person thinks that they are suffering from a heart attack! Gallbladder disease is when it becomes thick and the gallstones block the passing of bile. Gallstones can also form because of too much cholesterol in the diet or the hormone estrogen and the larger ones can cause pain and get in the ducts. Generally, it is believed if the attack follows the consumption of a heavy meal, and causes deep pain in the diaphragm area radiating to the right shoulder, one can suspect the involvement of the gallbladder.

Gallbladder problems often require that the organ be removed. If you are trying to avoid that or have already had it removed, you need to really watch what you eat. The biggest thing to avoid is fat. Fat is the primary material that bile attacks. There are many kinds of gallbladder pain and the severity is determined by the cause. The pain is usually the result of gallbladder stones that form and grow to a size larger than 8 mm in diameter. However, gallbladder sludge can also build up and travel to other parts where it causes pain. The presence of gallstones creates a possibility that cystitis, inflammation of the gallbladder, may develop. This can cause severe pain in the upper right abdomen and/or across the chest, possibly accompanied of fever, nausea, and vomiting.

The second type of pain grows over time. It is known as chronic. The gallbladder becomes diseased and inflamed. You start getting pain in the ribs and behind your shoulder blade on the right hand side of your body. If gallbladder problems continue to happen on a regular basis or become more severe in nature, then the treatment will depend on the cause. If the gallbladder is diseases, full of big gallstones or has cancerous polyps then the doctor will always choose to have it completely removed. If gallbladder disease symptoms persevere, are severe in nature or are chronic, then the only solution is to remove the entire organ. Since the gallbladder is non-vital and the bile can flow directly without it, there is no problem with the entire removal.

Having gallbladder disease does not mean that by the first symptom a surgical removal procedure will have to be performed. Most of the time there are no symptoms at all and in many cases the disease only results only in mild complaints such as gas, indigestion or nausea. A gallbladder can develop gallstones, polyps or even cancer.If you are having a gallbladder attack it can be caused when the the gallbladder becomes too thick and the gallbladder blocks the passing of bile. Gallstones can be caused by too much cholesterol in your diet or the hormone estrogen.

Usually there is a pattern to when gallbladder attack symptoms appear. They more often than not occur at night and also usually are triggered by certain foods. Especially after eating foods that are greasy or fatty you can expect an attack. Now there is a less invasive method of removal that involves lasers and most patients do not even have to stay overnight in the hospital.

Smart Cardio to Burn Fat

Myths Exposed and Little Known Secrets Revealed

 What do you think of when I mention cardio? An hour-long jog? A long, steady session on the exercise bike? Sweatin' to the oldies? If it's any of those things, you're doing cardio, but you're not doing smart cardio. There are lots of things you can do to burn calories, but if you're reading this article, you know that you want the calories you burn to come from fat, not lean mass. Traditional long, slow distance cardio burns muscle and fat pretty indiscriminately. In fact, if you do enough, you may find that your body burns muscle preferentially to ease the demands of doing so much aerobic work. That's exactly the opposite of what you want.

So how do you do cardio without sacrificing precious muscle? The answer is interval work. Definitely get yourself cleared by a doctor before jumping into intervals, because the whole idea is to rapidly and repeatedly raise your heart rate, alternating the high heart rate work with brief recovery periods. The optimal way to do interval work is probably to do walk back sprints. Sprint all-out for 15 seconds, then turn around and walk back to where you started. It should take about 45 seconds to walk back. Once you're back where you started, sprint for 15 seconds again. Do seven sprints your first week, and add one sprint per week until you're up to 20 sprints per session. If you're not up for sprints, you can approximate them on an exercycle or an elliptical machine. Simply go all-out for 15 seconds, then pedal or walk at a recovery pace for 45. The same build-up pattern applies.

In order to grasp this concept, you must first learn what your body burns for energy during strenuous exercise.  I call this energy body burn, "The Hierarchy Scale Of Energy."  The human body accesses certain nutrients for energy and does so in a precise manner every time.  This access order NEVER changes.  Let me repeat, NEVER changes.  This scale will alter the way you train forever!  Here it is.

   1. MUSCLE GLYCOGEN

(Sugar processed from carbohydrates and stored in muscle for contraction)

   2. LIVER GLYCOGEN

(Storage organ for excess glycogen; primary fuel source for brain function)

   3. BBA (BLOOD BORNE AMINO ACIDS)

(Branched Chain Amino Acids formed from protein)

4. FAT (Speaks for itself)

 ¨Notice where FAT is on the scale.  It is last!!!  It's the one thing we hate the most about our body and it's the one thing your body hates to give up.  The irony! 

 ¨It takes between 20-30 minutes to reach level #4, depending on the time of day you train and when you last ingested carbohydrates.

   Case#1: Come into the gym and jump on the cardio machine (pick your favorite).  Remember it will take 20-30 minutes to burn the muscle glycogen, liver glycogen and BBA before you reach fat.  In reality, the first 25 minutes of cardio is totally USELESS!! That is of course unless you actually enjoy doing cardio, simply for the love of doing cardio.  If you do, then I will affectionately call you NUTS!  Most people do it for fat loss, so why not make it the most effective?  To access fat, you now have to add an additional 20 minutes of cardio to get any results.  The entire cardio session alone will be around 45 minutes. Now you go to weight training.  But wait, where is your muscle glycogen (fuel for muscle contraction?) Right, it's gone!  The result; weaker workout, less muscle development, increased fatigue, over training, burnout, longer recovery and a slower metabolism.  In short, NO PROGRESS!  You get discouraged and quit training because your body is not changing.

   Case #2: Come into the gym and you jump on the cardio machine for 5-minutes to warm-up. Now you hit the weights.  Where is your muscle glycogen?  Right, ready to go for power and intensity resistance training.  Train for 30 minutes with weights and use up #1-3 on the Hierarchy Scale.  Result; increased strength, more lean muscle, less fatigue, no burnout, more intensity. Now begin cardio training.  What is the only available energy source left to burn for the cardio session since #1-3 are gone?  You got it, #4!!  Now you access fat stores immediately and only have to do 15-20 minutes of cardio to get the same benefits as 45 minutes in Case #1.  That means more fat loss! In short, PHENOMENAL PROGRESS!  You get psyched and train harder.

Which would you choose to do? I know my answer!

What is gout?
Gout is a metabolic disease, often familial, associated with hyperuricemia (high uric acid in blood) leading to acute arthritis, usually inflammation of the big toe, but may also effect other joints. Hyperuricemia can be because of over production or under excretion of uric acid, sometimes both.
Note : Some people have a high level of uric acid but do not form crystals or have gout. Also, rarely some peopel with a normal level or uric acid get gout attacks. However as a rule the higher the level of uric acid, the greater the chance of developing gout.

Why does uric acid build up?
It is often familial. Alcohol and certain medicines are important secondary causes. Alcohol ingestion promotes hyperuricemia by increasing urate production and decreasing the renal excretion of uric acid. Certain foods may tip the balance to raise your uric acid higher than normal. In particular, if you eat a lot of heart, herring, sardines, yeast extracts or mussles it may increase the level of uric acid. Some medicines such as diuretics and aspirin may raise the level of uric acid. How common is gout and who gets it?
Gout affects about 1 in 200 adults
Men are more commonly affected than women. Six out of 1,000 men develop gout, compared with one out of 1,000 women.
A first attack of gout typically develops in middle age, but it sometimes occurs in younger people. The first attack of gout in men most often occur between the age of 30 and 50. In women, it is rare for gout to occur until after menopause.
It tends to run in some families as there is family history of gout in about 1 in 5 cases. The genetic make up that you inherit from your family may be a factor in becoming an ‘under-excreter' of uric acid.

What are the symptoms of gout?
Gout usually occurs in ‘attacks'. An attack typically develops quickly over a few hours. Acute gouty arthritis is characterized by its sudden onset, frequently nocturnal, either without apparent precipitating cause or following rapid fluctuations in serum urate levels. Either increase or decrease in the serum urate level can precipitate a gouty attack. Common precipitants are alcohol excess (particularly beer), changes in medication that affect urate metabolism and fasting in hospitalized patients before medical procedures.

How is gout diagnosed?
Gout is usually diagnosed if you have the typical symptoms and a raised blood level or uric acid. If you think you have gout, see your general physician. He or she will ask about your symptoms and examine you, He or she may also ask about your medical history, especially family history. A blood test will measure the levels of urate in your blood but the level may also be high in healthy people who don't have gout. Fluid may be removed from your swollen joint with a needle. This usually causes no more discomfort than a blood test. If urate crystals can be seen in the fluid under a microscope, you have gout. If calcium crystals are seen, you have a similar condition called pseudo gout. An x-ray of your affected joint may be taken to rule out other medical conditions. It is also important to screen the kidneys in all cases with gount. Uric acid kidney stones are present in 5-10% of patients with gouty arthritis. Hyperurecemia correlates highly with the likelihood of developing stones, with the risk of stone formation receding 50% in patients with a serum urate level above 13 mg/dl. This and associated risk factors like hypertension, alcohol use and diabetes can lead to renal failure. Once diagnosed as a case of gout your base line uric acid should be maintained at 5 mg/dl and below.

Is gout serious?
A gout attack can be very painful. However, other effects from gout are uncommon. Joint damage may occur if you have recurring attacks. In a few people, uric acid cystals form kidney stones or may cause some kidney damage. Sometimes the crystals form bumps (tophi) under the skin. These are usually harmless and painless but sometimes form in awkward places such as the end of fingers. Tophi occasionally become infected.

Will obesity and consumption of certain foods lead to gout?
Obesity is common cause for gout according to current research. At least half of all gout sufferers are overweight, if not obese. The obese body puts a heavy load on the kidneys, above and beyond what they were meant to handle.
Research is also showing that it is not the protein that adds to the gout problem, but actually "purine rich foods". These include beans, peas, anchovies, liver, brains and (ironically enough) kidneys. Beer is also purine rich. It is the breakdown of the purine that causes uric acid to be formed. If the extra uric acid cannot be flushed from the system, they crystallize and lodge in joints, causing gout.
